Making a Referral

If you are homeless or at risk of homelessness, you can make a referral to our Community Outreach Team.

If you’re situation is an emergency and you have nowhere safe to sleep, please contact your local County Council.

Please read the instructions below before completing the referral form:

  • If you are making a referral on behalf of yourself, please complete our online self-referral form.
  • If you are making a referral on behalf of your client, please complete our online agency-referral form.
The information we collect is treated completely confidential and will only be used to evaluate the kind of assistance you need. Please be as honest as possible.

The Referral Process

Our referral process is a four-stage process as seen below.
1. Submit Referral:
Once you have completed your online referral, it will be sent for review to our Community Outreach Team.
2. Initial Interview:
Our Community Outreach Team will then get in contact with you and will arrange an initial informal interview which will help us to determine your needs and the service that will best suit those needs.
3. Interview & Assessment:
After the appropriate service has been chosen, an in-depth assessment will be carried out to ensure your needs will be met by the service.
4. Decision:
The final decision will be made as to whether North West Simon Community can offer you the correct support needed and you will then be notified of this.
